Primary Amenorrhea is defined as the absence of menses at age 16 when secondary sexual characteristics are present or absence of menses at age 14 when secondary sexual characteristics are not present. The prevalence of the condition at age 14 is 0.1-2.5% and the revalence at age 16 is 1-5%. Uterus,endometrial lining, ovaries, pituitary and hypothalamus must function properly and in a harmony for the routine menstrual cycle. Primary amenorrhea results from endocrinologic ethiologies in 40% and from developmental abnormalities in 60%. The history of the patient, hormonal parameters,diagnostic tools (such as ultrasound and magnetic resonance imaging) and laparoscopy can all be useful for the diagnosis of the condition. The answers of 3 questions are important for treatment in these patients. One of them is the menstruel flow, the other is probability of pregnancy and finally sexual intercourse. Additional anomalies must be investigated when the underlying pathology is developmental anomalies. Treatment modalities must be discussed with the patient and her family.The important point is to identify the underlying pathology and to discuss the condition in terms of treatment, sexual intercourse, concieving pregnancy and prognosis. In necessary cases the patients must be consulted with a psychiatrist. (Turk Arch Ped 2011; 46 Suppl: 92-6)


Primer amenore

Primer amenore, normal sekonder seksüel gelişim varlığında 16 yaşına kadar ya da yokluğunda 14 yaşına kadar adet görememe durumu olarak tanımlanır. Primer amenoreli olgularda sekonder seks karakteri yoksa 14 yaş civarında sıklık %0,1-2,5, sekonder seks karakterlerinin varlığında ise 16 yaş civarında sıklık %1-5 olarak karşımıza çıkmaktadır. Bir genç kızın adet görebilmesi için, uterus ve uterus içinde fonksiyonel endometriyum, overleri, hipofiz ve hipotalamusunun olması ve bu organların düzen içinde fonksiyon görmesi gerekmektedir. Primer amenore etyolojisinde %40 endokrin anormallikler, %60 ise gelişim anomalileri sorumludur. Tanıda; anamnez, hormon analizleri, görüntüleme yöntemleri (Ultrason ve Manyetik rezonans) ve laparoskopiden yararlanılır. Primer amenoreli olguda tedavide 3 sorunun yanıtı önemlidir: 1. Adet görüp göremeyeceği, 2. Gebelik olasılığı, 3. Cinsel ilişki. Özellikle gelişim anomalilerinin saptandığı olgularda ek sistem anomalileri araştırılıp, tedavi seçenekleri hasta ve ailesi ile tartışılmalıdır ve hastaya mevcut patolojinin üreme ve/veya cinsel fonksiyonlar ile olan ilişkisi anlatılmalıdır. Gerekli durumlarda primer amenore saptanan ergenlere psikolojik destek verilmelidir.(Turk Arş Ped 2011; 46 Suppl: 92-6)
