Methmoglobinemia secondary to topical anesthesic cream: a case report

When an infant presents with cyanosis which is not associated with respiratory distress, methemoglobinemia should always be suspected. Most of the acquired methemoglobinemia cases are due to local anesthetics in clinical practice and the methemoglobinemia level can be potentially harmful. Here we report a 36-day old infant with % 54 of methemoglobin level after the use of a local anesthetic agent for circumcision. Clinicians and parents must be aware that a topical anesthetic cream has this potential adverse effect. More studies are needed before routine use of topical anesthetic cream.


Topikal anestezik krem uygulamasına sekonder methemoglobinemi: olgu sunumu

Solunum sıkıntısıyla ilişkili olmayan siyanoz ile başvuran süt çocuğunda methemoglobinemi mutlaka düşünülmelidir. Pratikte kazanılmış methemoglobinemilerin çoğu lokal anestezik kullanımına bağlı olup methemoglobinemi seviyeleri potansiyel olarak zararlı olabilir. Burada sünnet için kullanılan lokal anestezik madde sonrasında methemoglobin düzeyi % 54 olan 36 günlük süt çocuğu sunulmuştur. Klinisyenler ve ebeveynler, topikal anestezik kremin bu potansiyel yan etkisini bilmelidirler. Topikal anestezik kremlerin rutin kullanımı konusunda daha fazla çalışmaya gerek vardır.
